To make changes to your player's team kit information, please submit THIS FORM. Should our fulfillment partner have any questions about your form response, they will reach out to the email you provide.

Please note: Only forms submitted before your PGA Coach has placed their team kit order can be accomodated. If you submit the form after your Coach has placed your program's order, the edit cannot be made and your team kit will be shipped with the information you selected during registration. Once your items arrive, you will have the option to purchase replacements if you would like to.
